FAQs for Athletic Registration and Medical Forms

 

Q: Where do I go in Genesis to register my student-athlete for a sport?

A: Log into the Genesis Parent Portal and navigate to:

Forms → Forms Library → (Season) Athletic Forms/High School 2026–2027

Complete all required registration forms and upload the required medical documentation before the registration deadline.


Q: What forms do I need to bring to my student's physical appointment?

A: The Athletic Department provides the following forms for your student's healthcare provider:

If your student's physical examination is more than 90 days old, you must also upload a completed Health History Update Questionnaire (HHQ) to the Genesis Parent Portal. Student-athletes with asthma and/or severe allergies must also upload a current Asthma & Allergy Action Plan.


Q: I completed the registration and uploaded all of the medical forms, but Genesis still says my student is "Not Eligible." What should I do?

A: Nothing—unless you receive a message from our Athletic Trainer.

At the beginning of each sports season, our Athletic Trainer reviews all submitted medical forms on a first-come, first-served basis, with priority given to sports whose start dates are approaching first.

Until your student's medical paperwork has been reviewed and approved, Genesis will continue to display "Not Eligible."

If any information is missing, incomplete, or requires correction, our Athletic Trainer will contact you directly through the Genesis Parent Portal with instructions on what is needed.


Q: My student has not completed the ImPACT Baseline Concussion Test. Where do we go to complete it?

A: Once your student has been reviewed by our Athletic Trainer, you will receive a link to complete the ImPACT Baseline Concussion Test, if it is required.

Please note: The ImPACT Baseline Concussion Test is only required once every two years.


Q: My student is interested in participating in a specific sport. How do they get more information?

A: Each sport holds an Interest Meeting prior to the start of the season.

Students are notified of Interest Meetings through:

  • The daily Tiger Rundown morning announcements
  • Athletic bulletin boards throughout the school
  • The @tenaflyathletics Instagram page

Students may also contact the head coach directly. Coach email addresses are available in the Coaches Directory on the Tenafly Athletics website.


Q: What medical forms are required to participate in athletics?

A: The only medical forms that must be uploaded to the Genesis Parent Portal are:

No other pages from the physical packet need to be uploaded.


Q: How often do I need to submit the Preparticipation Physical Evaluation Medical Eligibility Form (PPE)?

A: The PPE is valid for one year from the date of your student's physical examination and must be renewed annually.

Example: If your student's physical expires in November, they cannot be medically cleared to participate in Winter or Spring Sports until a new physical has been completed and a new PPE has been submitted.


Q: How often do I need to submit the Health History Update Questionnaire (HHQ)?

A: The HHQ is required whenever your student's physical examination is more than 90 days old.

Because many student-athletes participate in multiple sports throughout the school year, it is considered best practice to submit an HHQ with every seasonal registration to help avoid delays in the medical clearance process.


Q: My child participated in a Fall sport. Do I need to register again for Winter or Spring sports?

A: Yes. Student-athletes must complete a new athletic registration in Genesis for each sports season (Fall, Winter, and Spring), even if they participated in a previous season during the same school year.

Each seasonal registration ensures that emergency contacts, permissions, and required medical documentation are current before participation.


Q: Who do I contact if I have questions about athletic registration or eligibility?

A: For questions regarding athletic registration, required forms, or general athletic eligibility, please contact the Tenafly High School Athletic Office.

For questions regarding medical clearance, missing medical forms, or medical eligibility, please contact our Athletic Trainer.


Q: Can my student participate in tryouts or practices while waiting for medical clearance?

A: No. Per NJSIAA and district policy, student-athletes must be medically cleared by the Athletic Trainer before the official start of the season so that they may participate in any tryout, practice, scrimmage, or contest. We encourage families to submit all registration and medical forms as early as possible to allow sufficient time for review before the season begins.
